As announced at Mass recently, St. Agnes Parish will be creating a Memorial Plaque to honor deceased parishioners and loved ones. This plaque will be displayed in the new Divine Mercy Chapel in the rear of church.
With a $100 donation, the names of your loved ones will be inscribed on a brass plate attached to the Memorial Plaque. Each contribution will be used to cover the cost of the plaque, brass plates, engraving, and a portion of the chapel construction costs.
Please complete the form below and return it to the parish office, or the Sunday collection basket, with your cash or check made payable to St. Agnes Parish. You may inscribe the plate with up to three (3) lines of fifteen (15) characters per line. Spaces, commas, and other characters count as one space each.
